When you arrive for your first visit, you will be asked to fill out some paperwork concerning your past medical history and current condition. You can download these forms, print them, and fill them out ahead of time if you wish.

In order to best meet your needs, a individualized, private examination will be performed, including a thorough and complete history, systems review, and specific tests and measures. The evaluation will yield a movement-related diagnosis and prognosis, identifying the cause of your problems, in order to help direct our interventions.

Depending on your needs, you may be referred to one of our professional affiliates.

• Plain films, MRIs, or other diagnostic imaging and/or radiology reports you may have
• A list of your current medications
• Healthcare insurance information
• Comfortable clothes
